Community Dietitian required

Working as a member of the Medicines Management team and with guidance from the Chief Pharmacist, the dietitian will: • Undertake a rolling programme of audit of oral nutritional supplement (ONS) usage and subsequently develop and introduce guidelines for appropriate prescribing • Identify and address issues arising across the primary - secondary care interface • Act as a resource for the primary care teams providing direct and indirect advice for the dietetic care of patients; this will include providing formal and informal training sessions.
